- payment of $1,326.27 overdue since 9 September 2026, reassessment decision not issued

Hello,

This concerns file [number] and the 2 letters your Ministry issued on 2 September 2026.

The first letter approves my application for financial assistance as of 5 June 2026 and sets the first payment of $1,326.27 for 9 September 2026. No payment has been received, 11 days after the date your Ministry set for itself. I require the date the payment was issued, the method, and the account or address it went to. Where no payment was issued, I require the name of the official who holds this file and the date the payment will issue.

The second letter states that the internal review is completed, that the original ineligible decision was incorrect, and that a new decision letter will be sent after reassessment. 18 days have passed with no such letter. I require the reassessment decision in writing and the date it will issue.

Both letters of 2 September 2026 are addressed to 1754 St. Joseph Boulevard, Orleans, and neither reached me. I learned of them on 17 September 2026, from your own release under request CSS-2026-2794. I require the addresses held on this file, the date each was entered and by whom, and written confirmation that every further communication goes to [email].

On 17 September 2026 your Ministry wrote that an approved application is administered by the municipality. The approval is dated 2 September 2026. I require the date this file was transferred to the City of Ottawa, the office that received it, and the official there responsible for the payment.

Your Ministry refused this application on 18 June 2026, on 15 July 2026 and on 30 July 2026, and then found on 2 September 2026 that the original decision was incorrect. Entitlement runs from 5 June 2026. Nothing has been paid for any month since that date. I require the total owing to the date of your answer, broken down by month, and the payment date for each amount.

The appeal filed with the Social Benefits Tribunal on 28 August 2026 is before that Tribunal, and an application for mandamus on emergency assistance file 10000305517 is going to the Divisional Court. Your answer, or your silence, goes into both, and to the Ombudsman of Ontario, whose office holds this file. I require your answer within 5 business days.

Respectfully,

- shelter allowance of $390 a month not paid since 5 June 2026, arrears $1,508.00

Hello,

This adds one figure to my message of today on file [number], and it is your own.

Your letter of 2 September 2026 sets the first payment at $1,326.27. That number is the basic needs allowance and nothing else. $343 for 26 days of June, from the approval date of 5 June 2026, is $297.27. $343 for July, August and September is $1,029. The sum is $1,326.27, to the cent, and the shelter allowance appears nowhere in it.

I pay $390 a month for my housing, which is the maximum shelter amount payable to a single recipient under Ontario Works. At the time of application I was in an emergency shelter, and the SADA summary in my file records housing cost as not applicable. That entry has not been updated since, and your Ministry approved the application without asking about it.

The shelter allowance owing from 5 June 2026 to 30 September 2026 is $338.00 for June and $390 for each of July, August and September, being $1,508.00. Together with the basic needs allowance, the entitlement for that period is $2,834.27. Your Ministry has paid nothing at all.

I require the shelter allowance to be entered on this file at $390 a month with effect from 5 June 2026, and I require the arrears of $1,508.00 to be paid with the $1,326.27 that is already 11 days late. Where your Ministry declines, I require the provision of Ontario Regulation 134/98 it relies on, in writing, so that it goes before the Social Benefits Tribunal with the rest.

Respectfully,

Danilo Dzyuban

- the section 40 (2) (a) determination for June and shelter arrears of $1,352.00

Hello,

The shelter component on this file is governed by section 40 (2) (a) of Ontario Regulation 134/98, and the figure it produces is already on the record.

Catholic Centre for Immigrants Ottawa confirmed in writing on 7 August 2026: enrollment at St. Joseph Transitional Housing since 17 June 2026, monthly contribution $390, and outstanding $182 for June and $390 for July. Nothing has been paid on that account since.

Section 40 (2) (a) does not prorate shelter in the month of the effective date. The amount payable is the lesser of one full month of shelter cost and the shelter costs that remain unpaid on that date, which is $182 for June. Paragraph 3 of section 41 (1) and subsection 42 (2) then carry $390 for each of July, August and September.

The shelter arrears to 30 September 2026 are $1,352.00. With basic needs of $1,326.27, the entitlement for the period is $2,678.27, and the amount paid on this file is nothing.

I require 3 things. A determination under section 40 (2) (a) for June in writing, with the figure and the calculation. The arrears of $1,352.00 paid. The shelter line entered into the monthly calculation from 5 June 2026 and carried forward, so that the October payment contains it.

A request for internal review of the decision of 2 September 2026 on the shelter allowance was filed on 4 September 2026. The 30 days in which your Ministry must give the results expire on 4 October 2026. Silence on that date is itself appealable, and it goes to the Social Benefits Tribunal with the rest of this file.

Respectfully,

Danilo Dzyuban

- the payment of 9 September 2026 is basic needs alone, shelter stands at zero

Hello,

The payment of $1,326.27 reached me on 9 September 2026, and it settles what that figure is made of.

$343 for 26 days of June, from the effective date of 5 June 2026, is $297.27. $343 for July, August and September is $1,029. The total is $1,326.27, to the cent. Every line is basic needs under paragraph 1 of section 41 (1). Paragraph 3 of the same subsection, the shelter allowance, was paid at zero for every month.

Catholic Centre for Immigrants Ottawa confirmed in writing on 7 August 2026 that my contribution is $390 a month, that enrollment began on 17 June 2026, and that $182 for June and $390 for July stood unpaid. That debt is still open and it grows by $390 a month.

The shelter arrears to 30 September 2026 are $1,352.00: $182 for June under section 40 (2) (a), which does not prorate shelter in the month of the effective date, and $390 for each of July, August and September under subsection 42 (2).

I require the arrears of $1,352.00 paid, the determination under section 40 (2) (a) for June in writing, and the shelter line entered into the monthly calculation from 5 June 2026 and carried forward, so that the October payment contains it.

The request for internal review of the decision of 2 September 2026 on the shelter allowance was filed on 4 September 2026. The 30 days for the results expire on 4 October 2026, and silence on that date goes to the Social Benefits Tribunal.

Ontario Works - approved 2 September 2026, administered by the City, nothing paid

Hello,

This concerns the Ontario Works file of Danilo Dzyuban, and emergency assistance file 10000305517.

On 2 September 2026 the Ministry of Children, Community and Social Services approved this application as of 5 June 2026 and set the first payment of $1,326.27 for 9 September 2026. On 17 September 2026 the same Ministry wrote that where an application has been approved, Ontario Works assistance is administered by a municipality, and that records of such files are held there. The municipality is the City of Ottawa. No payment has been received.

I require the date on which the City received this file, the office and the official who hold it, and the date on which the City will pay the amount owing from 5 June 2026, broken down by month.

On 4 September 2026 your office wrote that eligibility was not formally determined. That was written 2 days after the Ministry approved the application with effect from 5 June 2026. I require either a written correction of that statement, or the record the City relies on to maintain it, identified by author and date.

Emergency assistance file 10000305517 still has no written decision. A written demand for that decision was made on 27 August 2026, your office refused it in writing the same day, and repeated the refusal on 4 September 2026. Both refusals rest on the same 2 propositions: that the process was not completed, and that eligibility was never formally determined. The Ministry determined eligibility on 2 September 2026, with effect from 5 June 2026, and it did so on the same application material the City held. I require the written decision on that application, with reasons and with the provision of Ontario Regulation 134/98 relied on.

An application for mandamus on file 10000305517 is going to the Divisional Court, and the Ombudsman of Ontario holds the record on this file. Your answer, or your silence, goes into both. I require your answer within 5 business days.

Respectfully,

Danilo Dzyuban

Ontario Works - shelter allowance of $390 a month not paid since 5 June 2026, arrears $1,508.00

Hello,

This adds one figure to my message of today on, and it comes from the Ministry's own letter.

The first payment set on 2 September 2026 is $1,326.27. That number is the basic needs allowance and nothing else. $343 for 26 days of June, from the approval date of 5 June 2026, is $297.27, and $343 for July, August and September is $1,029. The entitlement for that period, both components together, is $2,834.27. Nothing has been paid.

The Ministry wrote on 17 September 2026 that an approved file is administered by the municipality. This file was approved on 2 September 2026. I require the City to enter the shelter allowance at $390 a month with effect from 5 June 2026, to pay the arrears of $1,508.00, and to state the date on which it will do so.

Where the City takes the position that it does not hold this file, I require that position in writing, naming the office that does hold it. That answer goes to the Social Benefits Tribunal, to the Ombudsman of Ontario and into the application before the Divisional Court, alongside the Ministry's statement that the municipality administers approved files. I require your answer within 5 business days.

Ontario Works - the section 40 (2) (a) determination for June and shelter arrears of $1,352.00

Hello,

The shelter component on this file is governed by section 40 (2) (a) of Ontario Regulation 134/98, and the figure it produces sits in a document written by the housing operator.

The debt stands in my name with the operator, and it grows by $390 every month this file is left as it is.

Section 40 (2) (a) does not prorate shelter in the month of the effective date. The amount payable is the lesser of one full month of shelter cost and the shelter costs that remain unpaid on that date, which is $182 for June. I require the City to pay the arrears of $1,352.00, to enter the shelter line from 5 June 2026 and carry it forward, and to give the determination under section 40 (2) (a) for June in writing. Where the City says the file is not its own, I require that in writing, naming the office that holds it. I require your answer within 5 business days.

Respectfully,
