Addition to the open file - the City's refusal of 27 October 2026 on the application the City told your office did not exist - Danilo Dzyuban

Danilo Dzyuban, Ontario Works. This addition goes into the open file on the City of Ottawa and Emergency Assistance application 10000305517.

On 20 August 2026 your office wrote to me that Sylvie Bissonnette, Coordinator, Employment and Social Services, had explained that I "had not had an appointment / intake application for the Emergency Assistance application."

Attached is the City's decision letter dated 27 October 2026, signed by Michelle Lamonde for the Ontario Works Administrator. It states: "You submitted a request for Emergency Assistance on July 29th, 2026. We reviewed your request and it was determined on July 30th, 2026 that you did not qualify for Emergency Assistance."

The City's own letter confirms the application, its date and a determination on it. The account the City gave your office on 20 August 2026 was false. The City gave that account to an Officer of the Legislative Assembly during an inquiry into that application, and your office relied on it.

The letter of 27 October 2026 also states: "a review of the conditions set out in Section 56 of Ontario Regulation 134/98 ... was not undertaken." The determination the City now asserts for 30 July 2026 was never made under section 56, and it issued 89 days later, after service of my application for judicial review in Divisional Court file DC-26-00003254.

Your office is to investigate the account given by Sylvie Bissonnette on 20 August 2026 and the letter issued by Michelle Lamonde on 27 October 2026 as false information provided to the Ombudsman. State in writing by 31 October 2026 that the investigation is opened and who conducts it.
